Services

Parent Care Management Services (PCMS) offers a wide range of services for the geriatric client and his/her family. At the start of our term of service with a client, we conduct a full assessment of the individual, including physical, social, psychological parameters and also his/her ability to perform activities of daily living. To do this, we consult with family members, current caregivers, physicians, and other professionals. We then prepare a report, reviewing our findings and making recommendations for care provisions, options and costs. Clients may then choose whether they would carry out the recommendations or hire our staff to coordinate ongoing services.

We provide continuity and consistency of services to assist with in-home care or placement, as necessary. We coordinate appropriate services and monitor their effectiveness for your loved one. We work to avoid potential problems and conserve financial resources.

Services include:

• Screening, setting up, training, coordinating, and monitoring competent in-home help
• Designing personalized care plans and records/logs to ensure compliance with care plan
• On-going assessment of physical and mental health by periodic home or facility visits; regular phone calls for reassurance and problem resolution.
• Crisis management and early intervention to minimize deleterious effects
• Reporting to family and/or trust officer any change in condition or concerns
• Assessing current medical care and insurance coverage; advocating for the client as needed
• Assistance in obtaining available government and private entitlements
• Assist client and families in preparing advanced directives, power of attorney for health care documents, etc.
• 24-hour on-call coverage for emergencies

CONSULTATION SERVICES

Our consultation services may be all you need to address individual concerns. We are available to consult with you on a one-time only or a recurring basis. We are also available to meet with the entire family to assist in resolving conflicting views on care needs.

IN-DEPTH ASSESSMENT

We conduct an in-depth individualized assessment of the physical, social, psychological and financial strengths and needs of the client. We assess the safety of the home or facility, and the client's ability to manage the activities of daily living.

We consult family members, current caregivers, physicians and other professionals to determine current status and abilities. We provide a written review of our findings and recommendations. We discuss with you the best available options and resources. You may carry out the recommendations yourself or choose to have our staff provide and coordinate the ongoing services.
